Birth & Death Certificates

Yamhill County Vital Records maintains Birth and Death Records for 6 months from the date of birth or death for the purpose of providing certified copies. Copies can be ordered and obtained at the Public Health reception desk, located at 310 NE Kirby Street , McMinnville, OR 97128. There is a $25 fee per copy. Order forms can be filled out at our office or can be printed at home and brought into our office to order in person. The forms can also be used to request records from the OHA Center for Health Statistics via mail as listed on the form.

If certified copies are needed after 6 months from the date of birth or death, please contact Oregon Health Authority Center for Health Statistics at 971-673-1190.

Birth Records

A certified copy can be requested by the parent, maternal (mother) grandparents, paternal (father) grandparents (only if father is listed on birth certificate), siblings, legal guardian, legal representative, an authorized representative, or a government agency acting in the conduct of its official duties. ORS 432.380 (2)(c)(B)(ii). Photo ID of the ordering individual and proof of relationship is required to order a birth record. Death Records

A certified copy can be requested by the decedent's current spouse or domestic partner who is registered by the state, child aged 18 or older, parent, sibling, grandparent, grandchild, next of kin, legal guardian immediately before death, an authorized representative, a government agency acting in the conduct of its official duties, and an employee or agent of a funeral home. ORS 432.380 (2)(c)(B)(iii).  Photo ID of the ordering individual and proof of relationship is required to order a death record.
