YCSO Forms

In addition to being linked on pages that contain information about the program/service that utilize these forms, the current forms for use are:

  • Citizens' Academy Application (form is fillable so it may be completed electronically, but it requires a notary, so it must be submitted in person or mailed; emailed and faxed applications will not be accepted)

    • For information on our Citizens' Advisory Board, please see our Citizens' Advisory Board page. The last page of the Citizens' Academy Application includes the application for serving on our CAB. 

  • Civilian Ride Along Application (may be emailed, submitted in person, or mailed)

    • For more information on Ride Alongs, please see our Ride Along program page. 

    • At this time, Yamhill County Sheriff’s Office has suspended its ride-along program. In consideration of the safety of our interested citizens and YCSO deputies, we have placed this offering on hold while we work to revise the program to provide a better experience for our citizens. Ride-alongs will still be available for YCSO deputy candidates through the application process. Please continue to check back for any updates. 

  • Concealed Handgun License Application.

    • EFFECTIVE JUNE 26, 2020: To facilitate a smooth transition as we implement our new online application system, we are temporarily suspending acceptance of Concealed Handgun License applications until July 13, 2020. Please visit our CHL webpage on or after July 13th to submit your application through the new system.
       

 

  • Report a Crime Form (online form, submitted electronically through the website).

    • If the crime you are wanting to report occurred within the city limits of Newberg, Dundee, Carlton, Yamhill, or McMinnville, please contact that police department instead of submitting this form to Yamhill County Sheriff's Office, as we do not have primary jurisdiction in these cities.
