The Maternity Case Management program provides services to address the non-medical needs of pregnant clients who have risk factors which have been identified as potentially having an adverse effect on pregnancy outcome. The Maternity Case Management program provides an expansion of perinatal services to include management of health, social, economic and nutritional factors. Services include assessment of individual strengths and needs as well as referral and linkage to necessary services. The purpose of the program is to reduce the incidence of low birth weight infants and other unhealthy pregnancy outcomes.
