CaCoon- Coordinated Care Planning

CaCoon is a program for families that have children with special health needs. These children are at risk for developmental delay due to serious medical conditions, that may vary from heart malformations to genetic disorders. A public health nurse works with the family to assure that the families health, educational, social, and vocational needs are addressed in order to gain medical stability and optimal growth and development for the child. Families are assisted in learning new skills so they can become as independent as possible in caring for their children.


Services include:
  • Periodic home visits
  • Monitoring of child's growth and progress
  • Total health assessments
  • Referrals to local providers and service agencies

  • Identification of resources to meet special needs (including financial and educational)

  • Information and education on how to care for your child's special health needs

  • Encouragement and support
