TERMS AND CONDITIONS OF STANDARD PROBATION

1. You are to obey all laws, including all traffic rules and regulations, and you are to obey the reasonable and proper directions of your parent(s) or court-approved guardian(s) and probation officer.
   
   
2. You are to honestly and completely answer all questions asked of you by your parent(s) or court-approved  guardian(s), any probation officer or any law enforcement officer.
   
   
3. You are to report to your probation officer as directed, and you are to immediately report to your probation officer any arrests, law violations or law enforcement contacts.
   
   
4. You are to only live with your parent(s) or court-approved guardian(s); you are not to change your address or phone number without prior notice to a probation officer; and you are not to leave the State of Oregon without prior permission from a probation officer.
   
   
5. You are to attend school every day that school is in session, attend every scheduled class, and obey all school regulations.  You are to notify your probation officer by 8:30 a.m. on any school day that you will miss for any reason, and you are to notify your probation officer before you leave school if you need to leave early for any reason.  If you are home from school because of illness or suspension, you are not to leave your home that day or night except to keep a doctor's appointment.  You are not to be on any school campus where not enrolled without specific permission from the school's administration.
   
   
6. You are to seek and maintain regular stable employment as directed by your probation officer.
   
   
7. You are not to use or possess any alcohol, other intoxicants, illegal drugs, drug paraphernalia and/or any type of tobacco product.  You are not to be with anyone, other than your parent(s) or court-approved guardian(s), who is using or possessing any alcohol or tobacco product illegally, and you are not to be with anyone who is using or possessing other intoxicants, illegal drugs and/or drug paraphernalia, nor frequent places where such substances are kept or sold.
   
   
8. Your associates are to be approved by your parent(s) or court-approved guardian(s) and probation officer.  You are not to associate with any codefendants, gang members, probationers, any known criminals or delinquents.
   
   
9. You are to consent to search and seizure of your person, property, automobile, residence, or any container under your control at any time by direction of a probation officer with reasonable cause without benefit of a search warrant.
   
   
10. You are to consent to random and/or periodic tests of your blood, breath, or urine as directed by a probation officer.  You are not to possess or use any substance that would alter urinalysis.
    
    
11. You are not to posses any weapon of any description including but not limited to firearms, knives, nunchucks, or martial arts weaponry. You are not to possess any ammunition or weapon replicas.  You are not to possess any dangerous animal.  You are not to involve yourself in activities in which weapons are used, including but not limited to hunting or target shooting without prior permission from a probation officer. You are not to remain in any vehicle, dwelling or building wherein anyone possesses a weapon, ammunition, weapon replica or dangerous animal.
    
    
12. You are to obey any and all special terms and conditions of probation as ordered by the Juvenile Court.
    
    
13. You are to abide by all rules of any program or facility in which you are placed.
    
    
14. You are to carry valid picture identification at all times.
    

I have personally read, understand, and agree to abide by all of the preceding Terms and Conditions of Standard Probation.  I understand that my failure to comply with any one or more of the above terms could result in my arrest and/or return to court.
